Please bring these items to register your child at South Smiths Station Elementary.

  Electronic (Fillable) Registration Forms.  Please submit completed form(s) to your school's main office.  Parent or guardian of kindergarten students must complete both forms. In order to save the form with your data, save it to your computer first (before filling it out), then open it from your saved location on your computer.  Once filled out, choose "Save As" and give it a new name to keep a copy with all of your typed information.

Lee County School Systems Enrollment Requirements

1. WITHDRAWAL PAPERS from previous school.

2.BIRTH CERTIFICATE

3.STUDENT’S SOCIAL SECURITY CARD

4.Up-to-date ALABAMA IMMUNIZATION CERTIFICATE.

5.A copy of PARENT/LEGAL GUARDIAN’S DRIVER’S LICENSE, or other picture ID.

6.If the student does not live with parents, or if parents are divorced, then we must have a COURT DECREE declaring guardianship to the person registering the student.

7.PROOFS OF RESIDENCY showing you live in Lee County:
   {MUST PRESENT TWO (2) ITEMS LISTED BELOW}:

* APARTMENT/HOME LEASE AGREEMENT

* HOME PURCHASE AGREEMENT (HOUSE NOTE)

* Utility Bill – WATER, POWER, PHONE OR CABLE

(Cell phone bills are NOT accepted)

* PROPERTY TAX RECORD

* PARENT ID (ONLY IF IT HAS THE CURRENT ADDRESS)

***All information must show the Lee Road address and the Parent/Legal Guardian’s name.

8.If the legal parent or guardian registering the student resides in someone else’s home other than their own, a NOTARIZED RESIDENT AFFIDAVIT must be presented along with two proofs of residency.

9.Any student, who is re-entering after interrupted attendance, shall comply with the foregoing requirements.
